Diligently follow and execute the customized Plan of Care for each client, ensuring that their unique needs and preferences are met effectively.
2. Medication Reminders:
Provide timely medication reminders to clients, promoting medication adherence as prescribed by healthcare professionals.
3. Documentation Completion:
Accurately and comprehensively complete all necessary documentation, maintaining clear records of care provided, client interactions, and any significant observations.
4. Assist with Daily Activities:
Assist clients with various activities and daily life routines, such as bathing, dressing, grooming, mobility support, and other personal care needs.
5. Respect:
Diligently follow and execute the customized Plan of Care for each client, ensuring that their unique needs and preferences are met effectively.
6. Meal Preparation:
Skillfully assist with meal preparation, taking into consideration dietary requirements and preferences, and perform incidental household services as needed.
7. Supplies Maintenance:
Ensure that essential supplies are well-maintained and readily available, allowing for the seamless delivery of care services.
8. Monitoring and Reporting:
Actively monitor changes in the client’s condition, family dynamics, or evolving needs, promptly reporting such changes to the Team Lead for appropriate adjustments to the care plan.
